Working and assistance dogs

There are different registration requirements for working and assistance dogs. 

We offer a number of ways you can register your working or assistance dog. You can:

• apply through your existing online services account
• download the application form for a working or disability assist dog [DOCX 65 KB] and bring it into one of our service centres and pay the fee in person
• post your completed registration form to us; see Payments to select the option that suits you to make your payment, and send your completed form to:
